According to a recent article at NYTimes.com, gout is a disease that&#8217;s highly correlated to diet &#8230; in particular, alcohols and meats. And that&#8217;s why there can be a social stigma associated with the disease.</p>

You need an M.D., you’ll think. Maybe a CT scan or even an M.R.I.</p>
<p>But here’s what you may also need: some good P.R. Because if it turns out that your throbbing toe is caused by gout, you are going to have a lot of explaining to do as you limp through the day. What you have, any number of people will tell you knowingly, is the “disease of kings” — meaning, of course, gluttons.</p>
<p>While Henry VIII, said to be a gout sufferer, could do his explaining with a headsman for anyone bold enough to ask him about his diet, simpler folks may need to settle for a flack. And with more and more baby boomers likely to develop the disease as they get older, that publicist could become very busy indeed.</p>
<p>Let’s start with the basics: gout is a form of arthritis that occurs when uric acid, a waste product in the blood, forms crystals in the joints. The big toe is most famously affected. “A pox of this gout! or, a gout of this pox!” Falstaff says in “Henry IV.” “For the one or the other plays the rogue with my great toe.” (Note to ad agency: can we get a spokesman for the disease who is bit less cowardly, and perhaps not quite so rotund?) But gout can affect many other joints, including knees and elbows, causing pain, swelling and stiffness. Left untreated, it can cause permanent damage to the joints. Incidence of the disease is on the rise, and more than six million Americans have had gout attacks.</p>
<p>While everyone produces uric acid, in people predisposed to gout its level in the blood grows so high that the body cannot dispose of it properly. Imagine pouring packets of sugar into a glass of tea, said Dr. Herbert S. B. Baraf, managing partner of Arthritis and Rheumatology Associates in Maryland. “It can only hold so much in solution,” he said. And sooner or later, the sugar is going to start accumulating on the bottom of the glass.</p>

<blockquote><p><img class="alignleft size-full wp-image-1408" alt="Shoes that can cause bunions in Colorado Springs" src="http://www.painfreefootcare.com/wp-content/uploads/2013/04/shoes.jpg" width="300" height="182" />Sandal season will soon be upon us, and I’ve been perusing websites for spring shoes that provide enough comfort and support to walk around the city after work.</p>
<p>Flats are out, a foot specialist told me, as are sky-high heels. “You should look for a shoe with one-inch heel differential,” said Dr. Kenneth Leavitt, chief of podiatric medicine and reconstructive foot surgery at New England Baptist Hospital. That means a shoe with a two-inch platform at the sole should have a three-inch heel.</p>
<p>“Flat shoes stress the Achilles tendon and cause extensive pronation or flattening of the feet,” Leavitt explained. Surgery is sometimes warranted for severe, chronic foot pain to fix it.</p>
<p>High heels can cause persistent foot problems including corns, calluses or misshapen hammertoes — and can even cause stress fractures.</p>

<blockquote><p><img class="size-full wp-image-1400 alignleft" title="Toenail Fungus" alt="A person who has toenail fungus in Colorado Springs" src="http://www.painfreefootcare.com/wp-content/uploads/2013/04/toenail-fungus-non-drug-remedy.jpg" width="300" height="245" />One of the most effective treatments is the drug Lamisil, which is taken orally for 90 days and works for about two out of three people. But it has been associated with rare cases of liver damage and may require regular liver function tests, which can scare people away.</p>
<p>“The ‘risk’ of oral therapy has been largely overstated,” said Dr. Bryan C. Markinson, the chief of podiatric medicine and surgery at the Mount Sinai School of Medicine. “The truth is that after tens of millions of uses worldwide, this therapy is unequivocally safe.</p>
<p>Another option is treating the nail and surrounding skin with an antifungal nail polish. Prescription nail lacquers like ciclopirox have been clinically tested and approved by the Food and Drug Administration, and shown to be efficacious when used as directed, though not as effective as oral drugs; over-the-counter topicals are also available but have not been tested.</p>
<p>With the exception of skin allergies, topical antifungals carry little risk of side effects, but they remain unpopular with patients. “It’s not because they don’t work,” Dr. Markinson said. “It’s that they must be used for a minimum of one year on a daily basis to have any chance at a cure. Most patients lose enthusiasm after the first three months.”</p>
<p>The F.D.A. has approved laser therapy procedures for toenail fungus as well. One clinical trial involving a type of laser called Noveon found that nine months after treatment, just over a third of patients were cured. The downside? Laser therapy is costly — a single treatment can run $1,000 — and there have been some cases of burns on nails and the surrounding skin.</p>

<li>If you have diabetes or poor circulation in your feet, consult a podiatrist so he or she can recommend a customized pedicure that both you and your salon can follow for optimal foot health.</li>
<li>Schedule your pedicure first thing in the morning. Salon foot baths are typically cleanest earlier in the day. If you&#8217;re not a morning person, make sure that the salon filters and cleans the foot bath between clients.</li>
<li>Bring your own pedicure utensils to the salon. Bacteria and fungus can move easily from one person to the next if the salon doesn&#8217;t use proper sterilization techniques.</li>
<li>When eliminating thick, dead skin build-up, also known as calluses, on the heel, ball and sides of the feet, use a pumice stone, foot file or exfoliating scrub. Soak feet in warm water for at least five minutes, then use the stone, scrub, or foot file to gently smooth calluses and other rough patches.</li>

<h5>Don&#8217;ts</h5>
<ul>
<li>Resist the urge to shave your legs before receiving a pedicure. Freshly shaven legs or small cuts on your legs may allow bacteria to enter.</li>
<li>If you are receiving a pedicure and manicure, don&#8217;t use the same tools for both services as bacteria and fungus can transfer between fingers and toes.</li>
<li>Although certain salons offer this technique, don&#8217;t allow technicians to use a foot razor to remove dead skin. Using a razor can result in permanent damage if used incorrectly and caneasily cause infection if too much skin is removed.</li>
<li>Don&#8217;t round the edges of your toenails. This type of shape increases the chances that painful ingrown toenails will develop.</li>
<li>Emery boards are extremely porous and can trap germs that spread. Since they can&#8217;t be sterilized, don&#8217;t share nail files with friends and be sure to bring your own to the salon, unless you are sure that the salon replaces them with each customer.</li>

Reportedly, many individuals face such problems due to varied reasons. <a href="http://www.painfreefootcare.com/ingrown-toenails/">Ingrown toenails</a> can be treated by both surgical and non-surgical means. However, treatments of ingrown toe nails should be done immediately to avoid any complications. In case, the ingrown toenail is severe, the patient may have to undergo immediate surgical procedure for better management. Usually, most doctors prefer to take the initial surgical approach of partial avulsion. However, they may decide to go for lateral nail plate removal or a complete toenail removal depending upon the severity of your nail infection, inflammation, and pain. Sometimes, the ingrown toenail condition may reoccur despite of such surgical treatment. In such cases, destruction of the germinal matrix is conducted by a physician.</p>
<p>If you have ingrown toenail symptoms, you may want to try any of those self-care measures in order to manage your ingrown toenail symptoms. In fact, such self-measures are recommended before opting for the surgery.</p>
<h3>Self-care measures:</h3>
<p>Those with any symptoms of early ingrown toenail conditions may find their toe in a mildly inflamed condition. To avoid getting it worse, you can take some actions. Here are few important self-care measures:</p>
<ul>
<li>Soak your feet in Epsom salt water and follow good foot hygiene. Keep your feet dry and moisturized.</li>
<li>Follow the basic toenail trimming method and maintain it. Avoid trimming the nail in angular shapes as it may become ingrown.</li>
<li>Try using an oil-dipped cotton bud and gently push the skin away from the nail.</li>
<li>Always wear comfortable shoes that provide enough leg space for your toes. This helps in avoiding ingrown toenail conditions.</li>
<li>You can take over-the-counter painkillers if your ingrown toenail is painful.</li>
</ul>
<h3>Nail surgery:</h3>
<p>If your toenail doesn’t show any positive results in few days after initial self-care measures and it has excessive inflammation, try consulting a doctor. Sometimes, your toenail may become heavily swollen that may cause severe pain. If your toenail is infected, it may discharge certain fluid and pus. In case of infection, doctors may advise you to take oral antibiotics before the surgery. Depending upon the severity, your toenail may need to be partially or totally removed.</p>
<h3>Partial nail avulsion:</h3>
<p>If your toenail is not heavily infected, doctors will opt for partial nail avulsion. In this type of surgery, a part of your toenail is removed. This is the most common surgical procedure that can be used for treating ingrown toenails. The success rate of such surgical procedure is 98%. Your physician will give you local anesthetic medication for carrying out this procedure. After injecting the medication into the base of your toe, the surgeon will perform nail avulsion. In this case, the edges of your toenail will be cut away in order to make the toenail narrow. This in turn will allow the toenail to grow normally as it becomes less likely to dig into its surrounding tissue. After this procedure, phenol is applied to the affected area. This prevents the nail to grow back to its initial shape. In case of heavy infection, your doctor may advise you to take oral antibiotics.</p>
<h3>Total nail avulsion:</h3>
<p>Unlike the previous surgical method, total nail avulsion involves complete removal of the toenail. This surgical procedure is done to reduce the potential risk of redevelopment of an ingrown toenail. If the ingrown toenail is very thick and it presses the surrounding skin hardly, this procedure is recommended. Just as the partial nail avulsion procedure, the total nail avulsion method is also carried out using local anesthesia. As your toenail will be completely removed, there will be an indentation on your toe. Without the nail, your toe will function normally just as before. This procedure may require you to take oral antibiotics for few days only if there is any infection.</p>
<h3>After nail surgery precautions:</h3>
<p>Patients are advised to follow sterile conditions while dealing with their toenail. Clean your hands thoroughly before touching your toe. This helps in preventing infection. Try resting your foot for 1-2 days after the surgery. After the effect of anesthetic drug has worn off, you may feel pain in your toe. Your physician will advise you taking a painkiller for few days if you have severe pain. Keeping the area dry also helps in quick healing. Avoid using any ointment or moisturizer on the wound without consulting your physician. Using phenol can also help preventing the re-occurrence of ingrown toenail conditions. It is recommended to consult your physician if you find any sort of infection or discharge from the wound.</p>

In extreme cases, the infection can cause severe pain interfering with the ability to walk. Treatments range from topical and oral anti-fungal agents to surgery to remove the nail allowing direct contact with the infected tissue.</p>
<p>According to the <a href="”">American Podiatric Medical Association</a>, it is nearly impossible to avoid contact with the microorganisms which cause this disorder. This is because these fungi are one of the naturally occurring microorganisms found on the skin. Under the right conditions, any trauma that results in a crack can allow these organisms to breach the nail and begin to grow. Yet, there onychomycosis can be prevented.</p>
<h3>Improving Hygiene and Preventing Conditions that Cause Toenail Fungus</h3>
<p>There are a number of simple steps that can be taken to prevent onychomycosis. Two main methods are to keep feet as dry as possible since these fungi develop in warm, damp places, and avoiding trauma to the nails or nail beds. Other hygiene strategies include the following:</p>
<ul>
<li>Feet should be washed and dried completely before putting on socks and shoes</li>
<li>Antifungal products should be used regularly. Powders are beneficial after showers to absorb remaining moisture.</li>
<li>Wearing properly fitting shoes with enough space to allow the feet to breath to prevent moisture.</li>
<li>Shoes, socks and hosiery should be changed at least once daily.</li>
<li>Wearing socks made from synthetic material keeps dampness away from the feet better than natural fibers.</li>
<li>Avoiding walking barefoot especially in damp areas. This should also be practices at home since toenail fungus is contagious and often passed between family members.</li>
<li>Toenails should be clipped straight across ensuring that they do not extend beyond the end of the toes.</li>
<li>Avoiding use of nail polish preventing contact with chemicals they contain that damage the nails and trapping moisture beneath them.</li>
<li>Pedicure and trimming instruments should be sterilized every time they’re used.</li>
</ul>
<h3>Home Remedies to Prevent Nail Fungus</h3>
<p>Many prefer a natural approach to prevention. This inclination is not new and natural preventive measure for fungal infections have been used for centuries. Ancient Egyptians soaked their feet in Crocodile dung to prevent toenail fungus . (There’s no report of how well this worked so it’s best to try another strategy prior to locating a crocodile).</p>
<ul>

<li><strong><i>Carrier Oils</i></strong>–These thin oils such as sunflower and apricot, mixed with tea tree oil can be applied under the tips of the nails and around the edges before bed to prevent fungi from adhering to the nail.</li>
<li><strong><i>Methylsulfonylmethane</i></strong>–This substance ,which has a high concentration of sulfur, is believed to fight off fungi when applied directly to the nails</li>
</ul>
<p>While anecdotal reports suggest these methods are successful in preventing onychomycosis, no studies exist to date that prove the efficacy of these methods.</p>
<h3>Nutritional Supplements and Dietary Strategies</h3>
<p>Many physicians and naturopaths support the use of nutritional supplements and dietary changes to prevent toenail fungus. The list is extensive and several included here work by strengthening the immune system rather than acting directly on the microorganisms which cause toenail fungus. The most commonly mentioned supplements are listed below.</p>
<ul>
<li><strong><i>Acidophilus</i></strong>-Acidophilus increases the beneficial bacteria in the system, balancing out the harmful bacteria to prevent it from becoming too plentiful, which can lead to fungal infections.</li>

<li><strong><i>Essential Nutrients</i></strong>–Diets high in essential nutrients have been shown to strengthen the immune system and decrease sugars, which nourish the microorganisms that cause nail fungus.</li>

<p>Additionally, the following have been purported to have strong anti-fungal properties:</p>
<ul>
<li><i><strong>Chamomile Tea</strong></i></li>
<li><i><strong>Garlic Extract</strong></i></li>
<li><i><strong>Ginger Tea</strong></i></li>
<li><i><strong>Goldenseal</strong></i></li>
<li><i><strong>Lemongrass Tea</strong></i></li>
<li><i><strong>Olive Leaf Extract</strong></i></li>
<li><i><strong>Pau d&#8217;arco Tea</strong></i></li>
<li><i><strong>Turmeric Oil</strong></i></li>
<li><i><strong>Fresh Basil</strong></i></li>
</ul>
<p>Efforts to prevent toenail fungus are important as this condition can lead to additional health problems including the infection spreading to other parts of the body, the development of skin irritations and cellulitis, a bacterial skin infection which can result in pain, fever or chills, fatigue and muscle soreness. Additionally, onychomycosis can result in decreased self- esteem and a negative body image.</p>
<p>A strong immune system can provide resistance to many of the problems associated with onychomycosis and can fight off infections when the nails are exposed to fungi. Studies have shown that individuals most likely to develop toenail fungus are those with weakened immune systems. Some easy ways to boost the immune system include:</p>
<ul>
<li>Incorporating lean protein into every meal</li>
<li>Reducing stress by taking breaks and engaging in pleasant activities during the day</li>
<li>Consuming five cups of vegetables and fruit daily</li>
<li>Engaging in 20-30 minutes of physical activity daily</li>
<li>Cooking with healthy fats, such as canola or olive oil, to strengthens cellular functioning</li>
<li>Limiting alcohol intake to no more than five drinks per week since alcohol is toxic to the immune system</li>
</ul>
<p>While nail fungus is a common problem, it can be prevented. Practicing good hygiene will prevent an environment that is conducive to the microorganisms that cause nail fungus. Additionally, practices that strengthen the immune system such as consuming the proper nutrients and foods, exercising and limiting stress and alcohol can prevent an infection from developing by fighting off unhealthy microorganisms when they are encountered.</p>

<p><strong></strong>Tenosynovitis of the ankle is caused by inflammation of the tough membrane surrounding the tendons that passes through the joints on their way through the foot. If you are a runner, this is usually caused by trying to do too much too soon. In other words, trying to ramp up your training too quickly. The symptoms of tensynovitis include, pain when flexing or extending the foot, as well as a reddening of the skin around the tendon. Treatment options include, anti-inflammatory drugs, steroid injections, rest of the foot, immobilizing the ankle joint, and in some cases surgery.</p>
<h3>Peroneal Tendinitis</h3>
<p><strong></strong>The job of the peroneal muscle is to stabilize your foot while running or walking on surfaces that are uneven. This muscle can become sore when overused or from running too often on uneven surfaces. The pain is usually more noticeable on the outside of the ankle as well as the arch. Wearing shoes that have lost their cushioning can lead to peroneal tendinitis. Treatment options are more stretching and icing.</p>
<p>Running is a great way to stay in shape. However, running can be difficult on your body. These are some of the most common reasons that can cause ankle pain, while distance running. Stretching and proper fitting and supportive shoes can prevent a lot of ankle soreness. If soreness occurs, then icing and anti-inflammatory drugs can be used to treat symptoms. However, if you stretch and wear supportive shoes, you should have no problem with ankle soreness while distance running.</p>
